This Toyota and Lexus door lock actuator is the front left (driver side on left-hand drive cars) unit, OE 69040-33221. Toyota used this actuator across a long list of cars, SUVs and vans in the 2000s, so it is one of the most common door lock repairs we see. If the driver's door will not lock or unlock with the remote, this is the usual fix.
